-Sonder Health Plans, a local Georgia health plan providing personalized and affordable Medicare Advantage programs to residents, announce today the addition of Southeast Medical Group to its provider network. As the state’s largest partnership of independent primary care physicians, the addition of the group substantially widens Sonder Health Plan’s coverage base and ensures members have ample access to foundational care to drive better outcomes.

U.S. healthcare initiatives have identified access to primary care as essential to achieving the aims of value-based care and advancing key industry priorities such as social determinants of health (SDOH). A growing body of evidence underscores the relationship between high-quality primary care and the ability to address key industry priorities such as health equity, mental and substance abuse prevention and management of chronic conditions.

"As a local health plan, Sonder recognizes the importance of having an expanded network of primary care physicians as the basis for improving outcomes and expanding the goals of value-based care in Georgia," said Suzanna Roberts, CEO of Sonder Health Plans. "Our team prioritizes building strong local physician relationships, and we look forward to a long-term partnership with Southeast Medical Group."

Based in Atlanta, Sonder Health Plans recently announced an eight-county expansion and now serves members in 12 metro-Atlanta counties. The health plan’s benefits are comprehensive and uniquely designed to address the needs of Georgia seniors. Sonder’s innovative benefit plan offerings provide $0 copays for insulin, primary care and specialists visits. All of their benefit plans offer programs to help meet the everyday needs of seniors such as transportation services for doctor’s visits, over the counter and grocery card benefits.
