Lenbrook is partnering with Emory Integrated Memory Care (IMC), a nationally recognized primary care practice for people living with dementia, to address a growing resident need for specialized onsite care.

Beginning June 20, Lenbrook Residents can now participate in the IMC in Community program, which provides dementia-sensitive, comprehensive care directly to senior living communities. 

“Our goal is to provide opportunities where individuals can pursue connections, well-being and fulfillment – wherever they live within our community,” says Felecia Sveda, chief operating officer of Lenbrook. “We are excited to partner with Emory to bring its IMC program and offer specialized dementia-sensitive primary care right here at Lenbrook. This service makes it easier for residents to get expert, compassionate dementia care without leaving the Lenbrook community.”

An IMC nurse practitioner will create a tailored treatment plan and provide in-home medical visits to participating residents, in coordination with Lenbrook’s clinical and interdisciplinary wellness teams. The participants will also benefit from:

• Access to an off-site team of nurses, geriatric psychiatry services, and a social worker with dementia expertise

• Weekly 1:1 in-home visits with a Dementia Care Assistant for tailored, cognitively stimulating activities

• An 24/7 after-hours line to providers for urgent medical concerns

• Mobile phlebotomy and imaging as needed

• Care coordination with specialists and related services

In addition, family members of enrolled residents can engage with IMC support groups and educational classes. Clinical social workers will also serve as members of the care team, able to conduct psychotherapy sessions with family caregivers.

Lenbrook Residents in Independent or Assisted Living with any type or stage of dementia are eligible for the IMC program. The IMC services are offered for a monthly fee with no long-term commitment, providing flexibility and peace of mind.
